An integrated defectological approach to assessing the baseline status of patients with the sequelae of traumatic brain injury: Developing professional competencies in students via service-learning

Introduction. Traumatic brain injury (TBI) ranks among the most prevalent neurological conditions, often resulting in persistent deficits across cognitive, speech, emotional-volitional, and motor domains. Comprehensive baseline assessment of individuals with TBI sequelae is essential for devising effective remedial programs. Diagnosing post-TBI impairments is a multifaceted, multilevel process lacking a unified methodological framework; medicine, psychology, and pedagogy each rely on its discipline-specific diagnostic tools and methodologies. This knowledge fragmentation necessitates targeted pedagogical guidance to transform students’ disparate theoretical concepts into a cohesive professional vision required of defectology specialists. The service-learning approach facilitates this transformation, while practical immersion in the multidisciplinary diagnostic process transcends sporadic introductory placements, operating as a mechanism for conscious competency development.
Materials and Methods. This study analyzed medical records to characterize injury mechanisms, incorporating parameters such as TBI type and nature, lesion localization, severity, and the course of the baseline period. Supplementary data encompassed CT/MRI findings, surgical interventions, pharmacotherapy, and rehabilitation protocols. Psycholinguistic-pedagogical methods involved behavioral observation during interactions with peers, healthcare staff, and relatives, alongside express psycho-pedagogical screening; social determinants affecting baseline status were also evaluated. A pivotal pedagogical element was the involvement of speech-language pathology students in the baseline assessment process under institutional and clinical mentorship.
Results. Data analysis yielded a classification matrix comprising six defectological profiles, each defined by unique diagnostic criterion combinations, systematizing impairments in patients with TBI sequelae by injury type and severity. These profiles delineate defect structures: speech, praxis, and gnosis disruptions; swallowing and articulation; motor functions (gross and fine); cognitive processes (attention, memory, thinking); and sensory modalities (vision, audition, tactile perception). Implementing service-learning fostered integrative professional competencies in students, enabling a holistic perception of post-TBI psychophysical states — wherein speech impairments are interlinked with cognitive profiles, motor capacities, emotional states, and social contexts — and informed diagnostic and rehabilitative planning.
Conclusions. Baseline defectological assessment stratifies homogeneous groups by typological traits aligned to specific profiles. Derived insights furnish a holistic health profile for individuals with TBI sequelae, posit preliminary recovery hypotheses, and underpin corrective interventions alongside in-depth psycholinguistic-pedagogical evaluations. For speech-language pathology students, mastery of multilevel assessment transcends theoretical abstraction, forming core professional competencies.
